Overnight Field Trip Forms and Letter

The following form must be filled out in its entirety and must have a parent signature. Please make sure that there are emergency contacts and working phone numbers in the case of an emergency situation.

Medication Information form must be completed for ALL students going on the overnight trip.

This form also states that the parent understands the BASD board policy for administering , carrying, and possession of medications and that they understand the consequences if it is not followed. This form must be signed by a parent even if the student does not require medications on the trip. The student is not permitted to carry Over The Counter (OTC) medications- these will be carried by the school nurse attending the field trip Medical Information Form for Overnight Student Trips

Administration Form for Self Medication for Overnight Trips must be completed for ALL students going on the overnight trip even if they are not taking any medications. Medication Authorization form is to be completed for each medication that is needed daily- must be signed by a primary care provider and the parent. One is needed for each medication.

Only the following medications may be carried and self administered by the student with a valid prescription. All other medications must have an signed physician’s order and turned into the nurse that will be attending the trip to administer

Diabetes medication
Birth Control
Epi Pen
Asthma Inhalers

Medications must come in their original containers and there should only be enough for the trip dates plus 3 extra days in case of an emergency.
