Please refrain from emailing paperwork to counselors for approval. We must have original signatures on file. All forms have been updated as of May 2023, please only use forms linked below. Do not use older versions. 

PLEASE KEEP COPIES OF ALL PAPERWORK FOR YOUR RECORDS. 

• Parent Waiver (This form must be filled out BEFORE you can start your community service hours)
  • The Spanish version is available upon request.  
• Request to Volunteer & Agency Rubric
  • Please use these forms (2) when doing community service at an agency/organization not on our agency/organization list. Your hours will NOT be counted without this.
  • You cannot begin to complete hours until AFTER your site has been approved.
  • The rubric is to be filled out and turned in with the request to volunteer form.
    • Please ensure you are including the three required documents as noted on the rubric. (501c proof, mission statement, and insurance.)
